Patient Forms
Below you will find the patient forms to download and complete prior to your first appointment. Please return via email, or print and retun via fax or post if you are unable to email.
- Completing the Issues and Background (Form 2) saves time at the interview, expediting the process of arriving at a diagnosis and treatment plan, and potentially reducing the number of visits.
- Have your GP complete the information required in the Tests (Form 3) form.
- Have the Tests done in time so that the results will be available to me when you attend the interview.
- Ask your GP add copy to me for results when the tests are ordered.
- Please note, the forms are fillable PDF documents. To ensure we accurately record your details, use the ‘Save As’ function until you complete the forms and the ‘Save’ function when finished. If you use the ‘Export to PDF’ or ‘Print to PDF’ function, you will no longer be able to edit the forms and it will be harder to retrieve the information you have entered.
Thank you for completing these forms, it will make your sessions more productive.
If the information is not received at least one business day before the appointment, we may postpone your appointment to the next available date.

There are some important considerations for managing your health and taking medication following your appointment. In addition to the Prescription Information form below, the video below provides helpful guidance.

Travelling to Dr Perera and Parking
Dr Perera’s office is located at 140 Church Street. However, the entry is on Rule Street for which you need to come on Bromham Place and turn left when driving.
Please see the map below. This is the easiest approach due to the one-way streets and the fact that these are small streets. If you have any problems locating the suite, please call our receptionist.
Car – Parking
If you plan on driving, there is free on-street parking with time limitations. There is paid parking to the left on Bromham Place before you turn to Rule Street. Exit is via a token (current cost $4/-) obtainable from the suite receptionist.
Public Transportation
Public Transport Victoria allows you to map your journey here: https://www.ptv.vic.gov.au/journey/ This will provide you with the most current information to plan your journey whether you plan to travel by train, tram or bus.
myki
Please note, you will require a ‘myki’ to travel on trains, trams and buses in Melbourne. You can find the nearest place to buy a myki or buy a myki online at: https://www.ptv.vic.gov.au/tickets/myki/
Train
The closest train station to Dr Perera’s office is West Richmond (Mernda line and Hurstbridge line). It takes approximately 15 minutes to walk from the station to the office.

- Tram No. 12: St Kilda/Richmond North – Exit stop 21 (Church St/Victoria St – Abbotsford). Walk South along Church Street or catch Tram No 78 down Church street and exit at Stop 63
- Tram No. 48: Docklands/Balwyn North –. Exit at stop 18 (Church St/Bridge Rd – Richmond). Walk North along Church Street or catch Tram No 78 up Church street and exit at Stop 63
- Tram No. 75: Vermont South/Docklands – Exit at stop 18 (Church St/Bridge Rd – Richmond). Walk North along Church Street or catch Tram No 78 up Church street and exit at Stop 63
- Tram No. 78: Balaclava/North Richmond – Exit at stop 63 (Tweedie Place/Church Street)
- Tram No. 109: Port Melbourne/Box Hill. Exit at 21 (Church St/Victoria St – Abbotsford). Walk South along Church street or catch Tram No 78 down
Buses – Nos. 246, 302, 350 and 905
- Bus No. 246: Clifton Hill/Elsternwick – Exit at Cnr Wellington Parade/Hoddle Street (East Melbourne). Catch any tram East along Bridge Road and exit at stop 18 (Church St/Bridge Rd – Richmond). Walk North along Church Street or catch Tram No 78
- Bus No. 302: Lonsdale St (City)/Box Hill Station – Exit at Cromwell St/Victoria Pde (East Melbourne). Catch any tram East along Victoria Street and exit at stop 21 (Church St/Victoria St – Abbotsford). Walk South down Church street or catch Tram No 78
